If sending your application by email, please ensure that you sign the back page of the from and scan it in, as we cannot accept typed signatures.
Two forms of ID are required with this information and must show your name, date of birth, signature and current address of where you are living at the moment. Any official bank statements/letters must be current and within 6 months of the date on your application form.
Photocopies of your ID will be accepted and do not need to be verified (driving license, passport, utility bill, bank statement etc).
If you want your report sent to an alternative delivery address other than your current address, you will need to complete the alternative delivery address box when you complete your application.
Please provide an original signed letter of authority with your application if you authorise us to talk to another person about your application on your behalf such as a solicitor.
Please ensure you follow the instructions carefully that are on the form because your application will be rejected if it is not done correctly. It takes a maximum of 40 days for the application to be processed.
